Guardian Care Nursing and Rehabilitation Center provides a warm home-like environment for our residents who need to fully recuperate from a hospital stay before returning home, or those whose care must be monitored by a licensed nurse 24-hours per day. 

​We offer both semi and private accommodations.  Our resident bedrooms are spacious, and every room has its own private bathroom and shower, as well as cable TV, telephones, and Wi-Fi connectivity.

Subacute Nursing Care

Subacute Care treats residents that require round-the-clock short-term care until they are stabilized.
Our subacute services include, but are not limited to the following:

• Disease Management
• Diabetes Management
• Wound Care
• IV Therapies
• Enteral Feeding
• Pre- and Post-Operative Care
• Medication Management
• Congestive Heart Failure “CHF”
• Chronic Obstructive Pulmonary Disease “COPD”
• Tracheotomy

We offer Respite Care—A short-term admission to allow caregivers a weekend off or a much-needed vacation.

We contract with Hospice Agencies in the area to provide palliative care to residents with end-stage terminal illness
