At Guardian Care Nursing and Rehabilitation Center we understand nursing home placement can be a difficult decision for a Senior who knows they need more care than they can receive at home, or a child or other family member helping them to make the decision.

FAQs regarding coverage under Medicare Part A
Medicare Part A offers coverage for inpatient stays at a hospital and a certified skilled nursing facility.
MEDICARE PART A REQUIREMENTS:
A. Prior to applying for admission to a skilled nursing facility, you must have had a 3-midnight qualifying stay in an acute care hospital. This means you were admitted to the hospital in a patient bed (not just observed in the emergency room) for three consecutive midnights or more.
If you’re not able to be in your home during the COVID-19 pandemic or are otherwise affected by the pandemic, you can get SNF care without a qualifying hospital stay. |
B. A doctor completes a form, required by the state and federal government, Form 5000-3008 stating you need daily skilled care which must be given by, or under the direct supervision of a skilled nursing staff and or licensed therapists. If you are in a SNF for rehabilitation only, your care still is considered daily even if these therapy services are only offered 5-6 days a week.
The chart below shows for days 1 – 20, Medicare Part A will pay for all services; however, at day 21 – 100 days, you must pay a copayment amount set each year by Medicare. For 2021, that amount is $185.50. (If you have a Medicare Supplemental Plan, in addition to your Part A, it may cover the co-payment amount. We will be happy to check this out for you when you call.
Skilled Nursing Facility (per benefit period)
While Medicare covers up to 100 days of skilled nursing care if you continue to meet Medicare’s daily skilled need requirements, our goal is always to help you return to your highest level of independence as soon as possible.

A benefit period begins the day you are admitted as an inpatient in a hospital or SNF. The benefit period ends when you have not gotten any inpatient hospital care (or skilled care in a SNF) for 60 days in a row. If you go into a hospital or a SNF after one benefit period has ended, a new benefit period begins.
Skilled care is nursing and therapy care that can only be safely and effectively performed by, or under the supervision of, professionals or technical personnel. It’s health care given when you need skilled nursing or skilled therapy to treat, manage, and observe your condition, and evaluate your care.
Medicare-covered services include, but are not limited to:
• Semi-private room
• Meals
• Skilled nursing care
• Physical therapy (if needed to meet your health goal)
• Occupational therapy (if needed to meet your health goal)
• Speech-language pathology services (if they're needed to meet your health goal)
• Medical social services
• Medications
• Medical supplies and equipment used in the facility.
• Ambulance transportation (when other transportation endangers your health) to the nearest supplier of needed services that are not available at the SNF.
• Dietary counseling
