ESI_2227Guardian Care provides long-term residential care at our facilities to patients in need of skilled nursing assistance and therapy programs to maintain their highest physical, mental and psycho-social well-being. Our long term care includes a variety of services that give attention to the medical needs of patients through continuing care available 24 hours a day. We seek to assist the successful accomplishment of daily living activities and tasks of patients with chronic illnesses. Guardian Care aims away from traditional negative nursing home experience. Instead, we offer state-of-the-art care facilities that are clean, with care givers who are friendly and attentive.

Patients who make one of our facility’s their residence enjoy receiving the care they need as well as nutritious meals, comfortable rooms with electric beds and flat screen televisions, and a full activities calendar.

Guardian Care’s purpose is to provide its residents with high quality skilled rehabilitation and skilled nursing care in a manner that recognizes the value of human dignity at all stages of life.

Guardian Care is a 120 bed skilled nursing home facility located on 10 acres. On the property is also a 36 unit apartment facility for fully functioning seniors, who are able to live completely on their own. We are a “Five Star” rated facility, as rated by the Center for Medicare and Medicaid Services.

At Guardian Care, a team of certified professionals deliver your care with knowledge, compassion and consistency. Our team are from diverse backgrounds and speak multiple languages.

Our History

1958 - Opening

The facility was originally built by Florida Hospital in 1958 on land donated by the late Dr. Phillips and was operated as an acute hospital, until 1964, for the delivery of medical services to the black community.

1964 - New Ownership

In 1964 the South Atlantic Conference of Seventh-Day Adventists assumed its ownership and operation as a nursing home until 1968. It was at this juncture that Guardian Care, Inc., a 501(c)(3) corporation was formed to continue caring for the community’s long-term nursing needs.

1968-2015 - Excelling in Care

Today, Guardian Care is a Five-Star rated, 120 bed skilled nursing home facility located on the corner of John Young Parkway and Church Street. Guardian Care, the recipient of the Bronze Quality Award, offers skilled nursing and rehabilitation services including Speech, Occupational and Physical Therapy. 

2012-2015 - Received Excellence Awards

We are proud to announce our recent accomplishments, to include a deficiency free survey by the Agency for Healthcare Administration and the Bronze and Silver Awards from the American Healthcare Association.
